Terms of Service
The Practitioner:
•Sessions will be strictly confidential and conducted with professionalism and integrity.
•Offers therapy, coaching and nutritional therapy which is tailored to the individual client.
•Does NOT offer advice that is a substitute for general practitioner/medical advice or treatment.
•Refers client to a general practitioner or another practitioner where necessary.
•Respects the confidentiality of the client. Personal details are kept by the practitioner; in accordance with the code of conduct outlined by CNHC and the Information Commissioner’s Office. Information can only be passed on to another practitioner with your written consent.
The Client
•I understand that my plan is recommended for a limited period, specified by the Practitioner.
•I understand that it is my responsibility to inform the GP about any new supplement programme.
•I understand that it is my responsibility to inform the Practitioner about any health conditions, surgeries, medications, treatments, or supplements that I am taking.
•I understand that I cannot withhold any information which may affect the programme.
•I understand there are no specific guarantees on the individual success of treatments as each case is unique. With certain approaches such as CBT, EFT and NLP, I understand levels of success can only be determined by my susceptibility to these treatments and therapies, and my completion of actions agreed upon at the sessions.
•I understand that small ‘homework’ given by your practitioner must be completed in order to get the best out of your programme. If you are unable to carry them out, please speak to your practitioner.
•I understand that no responsibility or liability is accepted by Zen Mind for my actions or behaviour outside of sessions.
•If you are not on a package then payment will be due in advance for initial sessions and for follow-up sessions, payment must be arranged no later than 48 hours after you have received an invoice.
•I understand that there is a 48-hour cancellation policy in place because late cancellations cost precious time that could have been allocated to someone else in need.
